Key characteristics

• Licensing and oversight: Operates under Kentucky state regulations and relevant Medicaid waivers (such as the Supports for Community Living or other applicable programs), with oversight by designated state agencies to ensure quality, safety, and rights protections.

• Staffed supports: Trained direct support professionals (DSPs) or other qualified staff are present on-site or available according to the resident’s assessed needs and the program’s staffing model. Staff assist with activities of daily living, skill development, community integration, health monitoring, medication administration as authorized, and crisis support.

• Person-centered planning: Services are individualized based on assessments and a person-centered plan that reflects each resident’s goals, preferences, strengths, and required supports. Plans emphasize self-determination and community participation.

• Residential environment: Small group home residences. Living units provide private or shared bedrooms, common living spaces, and accessibility features as needed. The residence functions as the individual’s home rather than an institutional setting.

• Community inclusion: Supports focus on enabling residents to participate in community activities, employment, education, social connections, and civic life.

• Rights and protections: Residents retain civil and human rights; programs must follow state and federal rules on consent, confidentiality, grievance procedures, freedom from abuse and neglect, and restraint/seclusion restrictions.

• Funding: Funded through SCL Medicaid waiver and affordable monthly rent. Eligibility and service authorization depend on individual assessments and waiver enrollment.
